IMMUNIZATION REQUIREMENTS
Health Science Students are required to have the following immunizations and credentials in order to participate in the clinical practicum for their assigned program.
- BLS/CPR (American Heart Health Care Provider)
- Physical Exam (CCC Physical Exam Form)
- Annual TB Skin Test
- MMR
- Adult TD
- HEP B
- Varicella
- Criminal Background Check
- Malpractice Insurance-faculty in the clinical instructional role are covered under the students’ Blanket Malpractice Policy
